Hardware is rarely the problem.

Could you send the corresponding lspci output from when booting the
factory BIOS?

Seems there's a common problem with LB and PCI-PCI bridges. Also some
trouble on the M57SLI discussed right now.

Is your bridge also set to subtractive decode with the factory BIOS?

These two values look wrong.

This is definately not right.
